What To Bring

After a death has occurred, one of our Funeral Directors will contact you and arrange a convenient time for you to finalize the arrangements at our office.

In an effort to assist you in planning ahead, the following items should be considered for the meeting with a Funeral Director:

  • pdfInformation for Vital Statistics
  • Deceased's Social Insurance Number (required for Government notification)
  • Deceased's Alberta Health Care Number
  • Deceased's Driver's Licence Number, if applicable
  • Spouse's Social Insurance Number, if applicable
  • Marriage Certificate if there is a surviving spouse (required for CPP benefits)
  • A "VOID" cheque (if there is a surviving spouse) for direct deposit in the event the surviving spouse is eligible for a survivor's pension
  • A copy of the Power of Attorney or Enduring Power of Attorney document if the surviving spouse is not able to sign the application for Survivors Benefits from Canada Pension
  • Spouse’s Alberta Health Care Number if receiving benefits from Alberta Seniors Benefit Program

  • A copy of the deceased's will, if applicable
  • A full set of clothing including undergarments
  • The deceased's glasses, if applicable
  • The deceased's dentures, if applicable
  • A photograph for the obituary
  • Information about family and a brief biography for the funeral home to use in preparing/formatting the obituary for the newspaper
  • Photographs for the service folder (program)
  • Photographs for a video tribute (40 photos and 2 selections of music)
  • Pallbearers' names
  • The name of the person giving the tribute/eulogy
