After a death has occurred, one of our Funeral Directors will contact you and arrange a convenient time for you to finalize the arrangements at our office.
In an effort to assist you in planning ahead, the following items should be considered for the meeting with a Funeral Director:
- Information for Vital Statistics - Fillable PDF form (click on the orange link to access the document)
- Deceased's Social Insurance Number (required for Government notification)
- Deceased's Alberta Health Care Number
- Deceased's Driver's Licence Number, if applicable
- Spouse's Social Insurance Number, if applicable
- Marriage Certificate if there is a surviving spouse (required for CPP benefits)
- A "VOID" cheque (if there is a surviving spouse) for direct deposit in the event the surviving spouse is eligible for a survivor's pension
- A copy of the Power of Attorney or Enduring Power of Attorney document if the surviving spouse is not able to sign the application for Survivors Benefits from Canada Pension
- A SIGNED copy of the deceased's will, if applicable
- A hard copy of one piece of government issued identification (such as a driver’s license, passport, provincial health care card, etc.) belonging to the deceased individual
- Government issued photo identification (such as a driver's license, passport, etc.) of the Executor of the will or of the individual responsible for making funeral arrangements. Click on the orange link provided for the line of authority of 'who may control disposition' as defined by the Funeral Services Act - General Regulation, in Alberta.
- A full set of clothing including undergarments, if applicable
- The deceased's glasses, if applicable
- The deceased's dentures, if applicable
- A photograph for the obituary
- Information about family and a brief biography for the funeral home to use in preparing/formatting the obituary for the newspaper
- Photographs for the service folder (program)
- Photographs for a video tribute (40 photos and 2 selections of music)
- Pallbearers names
- The name of the person giving the tribute/eulogy
